      (Please fill out this physical form and NOT the LBHS school form. This is three (3) forms in one and must be notarized!)

• NOVICE ROWERS: Please complete forms 1 - 9.

• RETURNER ROWERS: Please complete forms 1-8 and 10.

Returners... please bring completed forms 1-8 & 10 to the Returner Rower Sign Up Day at the boathouse on Sat., August 14, 2010. We can notarize your physical form for you that day.

All forms must be completed and turned in within 14 days of joining LBRA.
No rower will be allowed to participate in practice after this date until all forms have been completed and submitted. Please turn in all forms to the membership chair person. Thank you for your cooperation on your prompt return of these forms.

For any questions, email the MEMBERSHIP CHAIRPERSON to make arrangements to turn in paperwork or have your rower place forms in the bins on the wall at the boathouse.
