      (Please fill out this physical form and NOT the LBHS school form. This is three (3) forms in one and must be notarized!)
  4. Lake Brantley Rowing Association Information/Questionnaire
  5. Lake Brantley Rowing Association Nametag Request
  6. Lake Brantley Rowing Association Fundraising Guidelines
  7. Lake Brantley Rowing Association Medical Information Release
  8. Lake Brantley Rowing Association Fees


ALL FORMS MUST BE COMPLETED AND SUBMITTED WITHIN 14 DAYS OF JOINING LBRA.
Rowers will not be allowed to participate in practice after this date until all forms have been completed and submitted. Please turn in all forms to the Membership Chairperson. Thank you for your cooperation on your prompt return of these forms.

Email the MEMBERSHIP CHAIRPERSON to make arrangements to turn in paperwork or have your rower place forms in the bins on the wall at the boathouse.
