Please print and fill out the following forms legibly and completely:
1. Lake Brantley Rowing Association Liability Release - Submit two (2) copies
2. Lake Brantley Rowing Association Academic and Conduct Code
3. Lake Brantley Rowing Association Physical, Medical and Emergency Release (Please fill out this physical form and NOT the LBHS
    school form. This is three (3) forms in one and must be notarized!)
4. Lake Brantley Rowing Association Information/Questionnaire
5. Lake Brantley Rowing Association Carpool Notice
6. Lake Brantley Rowing Association Nametag Request
7. Lake Brantley Rowing Association Fees and Fundraising for Novices
8. Lake Brantley Rowing Association Fees and Fundraising for Returners

Forms 1-7 for novice rowers and forms 1-6, and 8 for returning rowers must be completed and turned in within 14 days of joining LBRA. No rower will be allowed to participate in practice after this date until all forms have been completed and submitted. Please turn in all forms to the membership chair person before practice at the VFW, or contact her by phone to make other arrangements within the 14-day period.

The monthly General Meetings are held at 7:30 PM in the Media Center (unless otherwise posted on the door) at Lake Brantley High School. Meeting dates can be found here! See you there and at the finish line of every race! Welcome to the wonderful world of rowing!
